1. We love our puppies, and want to protect you (the buyer), the puppy and us the seller. We
ask that you give your new baby the same tender loving care that they received with us. We are
always here to help answer any questions you have. We want you to be 100% happy and have a
wonderful experience with your new puppy.
2. Puppy’s health is guaranteed to the best of our knowledge at the time of the sale and has
had a routine health examination by our licensed veterinarian. His/her last veterinary exam was on
____________________.
3. An up-to-date copy of the puppy’s medical history is enclosed in your Puppy Welcome Kit.
4. In the event that the puppy is going to be spayed/neutered, the Buyer agrees to wait until the
puppy is at least 6 months of age, due to the health concerns of putting a puppy under anesthesia
before it is 6 months of age.
5. To maintain the guarantee of this contract, the puppy must be vet checked within 1 week of
the purchase date. If this puppy is not vet-checked within 1 week of the purchase date, this contract
is null & void.
6. All of the puppies are AKC registered. If for some reason the papers on your new baby have
not been returned to us by the AKC when it comes time for your puppy to go home with you. We will
either forward the papers to you the moment they arrive or keep the puppy until they do. The choice
is yours.
7. It is understood that this puppy has not been purchased for resale or for use by any
wholesale operation.
8. Under no circumstances is this puppy to be sold, leased or given away to a pet store,
laboratory, research facility or animal shelter.
9. Buyer agrees that if for any reason, at any time, the new puppy does not work out, the puppy
is to be returned to the Seller/Breeder ONLY and never to a pet store, dog farm puppy mill,
wholesaler, agent or the pound. We will always welcome the puppy back, no questions asked.
10. It is further agreed that any and all efforts will be made to prevent any offspring of this puppy
from being sold to such parties (i.e., pet store, dog farm, puppy mill, etc.)
11. Buyer aggress that the puppy is part of your family and is not to be kept outside in the
elements, or on a chain…Shih-tzu dogs are house pets and are to be loved as you would love your
children.

a service fee of $50.00 for any returned check and/or money order, in addition to the purchase
price of said puppy.

1. Please do not send a deposit unless you are certain that you want to adopt a puppy from
Benjamin’s Babies.
2. If you decide to adopt a Benjamin’s Babies puppy you must:
a. Notify us by phone (443) 225-5298 or e-mail BenjaminsBabies@yahoo.com that you are
interested and sending a deposit.
b. Send a minimum $200/puppy deposit in the form of a check, money order or cashiers check
to: Benjamin’s Babies, 1120 High Street, Cambridge, Maryland, 21613. (Note: A $50.00 non-
refundable fee will be charged on any returned check or money order, in addition to the total
adoption price of the puppy).
3. We will mark the puppy “on-hold adoption pending” until your deposit is received. If your
deposit is not received within 72 hours of notifying us by phone or e-mail, the puppy will again be
available for adoption. Once your deposit is received, the puppy will be marked as “adopted”.
4. All payments must be paid in full before the puppy goes to their new home.
5. If you should change your mind on the puppy you have chosen, or come upon a financial
hardship, your deposit will be refunded in full, no questions asked. We want this to be a stress-free
purchase made out of love, not obligation. So if you cannot take the puppy for any reason, we
understand, and there will be no hard feelings. We just ask that you notify us as soon as possible. It
is our mission to have our puppies in loving homes, where they are wanted and where appropriate
vet care and food is not a hardship for anyone.
6. Please sign and include this form with your deposit. We cannot accept deposits without this
form.
